The best pillow for neck pain
If you wake with a stiff or aching neck several days a week and it eases within an hour of getting up, the cause is often what your head rested on for the previous eight hours. A pillow cannot cure a medical problem — but the wrong one reliably creates a mechanical one.
Why the pain happens
Your neck has a natural forward curve. When you lie down, a pillow either supports that curve or forces it out of shape. Hold any joint at the end of its range for eight hours and it will complain in the morning — that is what a too-high or too-flat pillow does to the small muscles and ligaments at the base of the skull.
The giveaway is timing. Pain that is worst on waking and eases through the morning points at your sleeping setup. Pain that builds through the day, or that came on after an injury, does not — see a doctor or physiotherapist for that.
Height matters more than firmness
Most people with morning neck pain buy a firmer pillow. That is usually the wrong variable. The problem is normally loft — the pillow's height under your head — and firmness only matters insofar as it keeps that height stable through the night.
- Too high — chin pushed towards the chest, back of the neck stretched all night. Common in back sleepers using a pillow bought for side sleeping.
- Too low — head tips back or drops towards the mattress. Common in side sleepers on a soft, collapsed pillow.
- Too soft — the right height when you lie down, wrong height an hour later once it has compressed.
What tends to work
A contoured cervical pillow — a shaped foam profile with a raised ridge under the neck and a lower hollow for the head — is the most reliable starting point for back and side sleepers. It sets the height for you rather than relying on you to find it.
Firm memory foam is the next best option. It holds loft better than fibre, which is the failure mode that puts most people back where they started within a few months.
What to avoid
- Stacking two pillows. It adds height but creates an unstable, sloping surface that shifts as you move.
- Sleeping on your front. It forces the neck into rotation for hours. If you cannot break the habit, use the flattest pillow you own, or none.
- Very soft down or fibre. It feels comfortable in the shop and gives you nothing by the middle of the night.
The mattress can be the cause too
